3 week old kitten care

3 week old kittens:
1. When do I start to offer them food, what kind, and how much?
2. When do they start using a litter box?
3. Do you recommend a book that would cover kitten care this young? Most books don't give information for younger than 6 weeks.
4. One kitten died yesterday. The vet did an autopsy and found milk in her trachea, but didn't mention it being in her lungs. If she aspirated, wouldn't milk be in her lungs? Also, how common is this cause of death?
